Nazareth Hospital Achieves Recertification for Joint Replacement, Stroke and Heart Failure

NORTHEAST PHILADELPHIA (November 26, 2012):  Nazareth Hospital hosted The Joint Commission recently for three separate one-day recertification surveys for the hospital’s total joint, stroke and heart failure programs. Nazareth has again earned The Joint Commission Gold Seal of Approval™ and two-year certification for all three programs. Disease-specific certification is awarded by The Joint Commission based on a program’s compliance with national standards, implementation of clinical guidelines, and achievement of excellent care outcomes.

“These national recognitions speak to Nazareth Hospital’s continuing commitment to ensure our community has access to high-quality care – emergency, cardiovascular, orthopedic,” said Nancy Cherone, Executive Director/Administrator, Nazareth Hospital. “The surveyors were extremely complimentary of our programs and our staff.”

Nazareth’s joint replacement program covers the complete continuum of care and includes community education, diagnostics, comprehensive preoperative education classes, expert surgical care, specialized post-surgical nursing care, nationally accredited rehabilitation and post-rehab outpatient physical therapy.

Nazareth Hospital’s stroke team offers an integrated approach to diagnosis, treatment and prevention of and education about stroke. Nazareth Hospital, one of America’s 100 Best Hospitals for Stroke Care according to HealthGrades, earned its second consecutive Get With The Guidelines Gold Plus Award from the American Stroke Association. The hospital also received the 2012 HealthGrades Stroke Care Excellence Award™. Evidence-based, consistently excellent and delivered with respect, Nazareth’s stroke program offers each patient a chance at a superior outcome: quick diagnostics, emergency treatment, specialty nursing care, and nationally accredited acute rehab.

Nazareth Hospital’s Heart Failure (HF) team received the Get With The Guidelines®–Heart Failure Gold Quality Achievement Award in 2012. The program emphasizes comprehensive patient education and the use of remote health monitoring technology to help patients manage the symptoms of their disease.
