Nazareth Hospital’s Heart Failure Team Receives Get With The Guidelines Gold Quality Achievement Award

NORTHEAST PHILADELPHIA (May 31, 2012): The American Heart Association’s Director of Quality Improvement, Tawny Jackson, came to Nazareth Hospital recently to present the Get With The Guidelines®-Heart Failure Gold Quality Achievement Award to Nazareth’s Heart Failure team.

The recognition signifies that Nazareth Hospital has reached impressive benchmarks in the evidence-based treatment of heart failure patients. The Get With The Guidelines program offers healthcare providers with proven tools and processes designed to improve outcomes for patients with heart failure. This high-tech, evidence-based approach enables Nazareth Hospital to improve the quality of care it provides heart failure patients, save lives and ultimately, reduces healthcare costs by avoiding re-hospitalization.  

Nazareth Hospital is nationally certified by The Joint Commission for heart failure care and offers patients with congestive heart failure access to a comprehensive continuum of care that includes cardiology and patient education, as well as telemonitoring and other home health services through Mercy Home Health.

According to the American Heart Association, about 5.7 million people suffer from heart failure.  Statistics also show that, each year, 670,000 new cases are diagnosed and more than 277,000 people will die of heart failure.
