Mercy LIFE is an approved PACE (Program of All-inclusive Care for the Elderly) program that helps older adults continue to be independent at home and in the community

• Provides quality medical, health and social services at home and at the Mercy LIFE Day Centers
• Develops a personalized long-term care plan for each participant
• Participants continue living at home while receiving services
• Serves older adults in North and South Philadelphia
