Nazareth Hospital’s Transitional Care Unit gets 5-star rating from the Center for Medicare & Medicaid Services

The Transitional Care Unit of Nazareth Hospital has been recognized by U.S. News & World Report for the outstanding 5-star rating the unit received from the Centers for Medicare & Medicaid Services (CMS).
The rating reflects excellent performance in three areas measured by CMS: health inspections, staffing and quality measures.
“This is a wonderful recognition of the dedication and expertise of our staff and the excellence of our program in helping patients bridge the gap between hospitalization and return to home,” says Rosemary Valentine, Director of the Transitional Care Unit at Nazareth Hospital.
Located on the 6th floor of Nazareth Hospital, the Transitional Care Unit allows patients to receive medically supervised therapy by highly trained physical, occupational and speech therapists, with the goal of maximizing safety and function before returning home.
