Community Paramedicine Program

 Easing Patients into Home Recovery

 

We understand that the transition home after being discharged from the hospital can be a challenging time for patients and caregivers. From filling prescriptions, to following medication instructions and scheduling doctor’s appointments, it can be a difficult and stressful adjustment. 

To help ease patients into home recovery, Mercy Fitzgerald Hospital’s Community Paramedicine Program provides patients with three home visits from a paramedic, to make sure they are on the right path to healing and recovery.  

 
First Visit

  • This visit will be scheduled before the patient is discharged from the hospital and will occur 24-36 hours after leaving the hospital. The visit will last approximately 45-60 minutes.

During the first visit, the Paramedic will:

  • Check vital signs and monitor weight.
  • Ensure that the patient understands discharge instructions and that prescriptions have been filled.
  • Verify that the patient has the correct medication(s), is educated on their medication(s) and is taking the correct dosage.
  • Ensure that any necessary follow-up appointments have been scheduled.
  • Conduct a home assessment to ensure the patient is living in a safe, clean and healthy environment vital to healing and recovery.
  • Make any necessary recommendations for additional Home Health Care. 

Second Visit

  • This visit will be scheduled during your first visit and will occur 24-36 hours after the first visit. The visit will last approximately 30-45 minutes.

During the second visit, the Paramedic will:

  • Check vital signs and monitor weight.
  • Answer any questions.
  • Provide the patient’s physician with an update on how the patient is doing. 

Third Visit

  • This visit will be scheduled during your second visit and will occur within one week of your second visit. The visit will last approximately 30 minutes.

During the third visit, the Paramedic will:

  • Check vital signs.
  • Answer any questions.
