European patients were denied surgery based on age and ejection fraction. 

"The fact that non-operated patients were older and more frequently had moderate impairment of LVEF (left ventricular ejection fraction) than operated patients suggests that the decision not to operate may NOT always be justified by the corresponding risk-benefit analysis of surgery."
