Mercy Home Health offers an in-home telemonitoring program to help heart failure patients maintain a healthier lifestyle, stay independent, and reduce Emergency Department visits and hospital admissions.
Using advanced equipment, the program monitors weight, vital signs and blood oxygen saturation daily.
The information is sent to a Mercy clinician, who can determine if follow-up care or actions are needed.
Patients are evaluated for inclusion into the program. Either or both the patient or caregiver must be able to perform the in-home monitoring independently.
