Mercy Fitzgerald Hospital offers a specialized center for the evaluation and treatment of sleep disorders. Using advanced equipment, the Sleep Disorders Center conducts sleep tests to correctly determine the cause of a person’s sleep disorder and how it can be treated.

Sleep Disorder Evaluation and Treatment Questionnaire

After you make your appointment, you will be asked to complete a brief questionnaire regarding your sleep and medical history. You should bring the completed questionnaire with you to your initial evaluation.

Initial Sleep Disorder Evaluation

During the first visit, you will be evaluated by sleep disorder physician. This includes a detailed discussion about your sleep problem and the information in the questionnaire. A sleep test is usually recommended.
