Mercy LIFE Medical Director Receives Third Board Certification

Mercy LIFE Medical Director and Mercy Health System Associate Chief Medical Officer Dr. Donna Brady Raziano recently received her third medical certification from the American Board of Internal Medicine. Her certifications include Internal Medicine, Geriatric Medicine, and the most recent, Hospice and Palliative Medicine. Dr. Raziano also has a Master's of Business Administration in Health Care Management from the University of Pennsylvania's Wharton School of Business and is a Fellow Diplomat of the American College of Physicians.

According to the American Board of Internal Medicine, maintenance of physician certifications promotes lifelong learning and the enhancement of the clinical judgment and skills essential for high quality patient care. The Mercy LIFE program, which is a nationally recognized Program for All-Inclusive Care for the Elderly (PACE), provides aggressive and preventive healthcare to some of the most complicated and fragile geriatric patients living independently in the community.

With the ultimate goal of preventing unnecessary hospitalizations and/or nursing home institutionalizations, Dr. Raziano is deeply committed to the PACE model of care and sits on the National PACE Association (NPA) Primary Care Committee and Research Committee. She was recently awarded the NPA Service Award for her leadership on the NPA Primary Care Committee and she also received the NPA Certificate of Appreciation from the NPA Quality Improvement Consortium.

The Mercy LIFE healthcare delivery model is designed to help participants of the program remain as independent as possible at home and in the community by providing all necessary medical, social, and supportive services within the home and at the Mercy LIFE Adult Day Centers. Through a multidisciplinary team approach, Mercy LIFE's healthcare professionals, who specialize in geriatric care, work with the participant and their family to evaluate and develop a personalized plan of care.
