Nazareth Receives PA-HEN Readmission Immersion Project Award
NORTHEAST PHILADELPHIA, PA (April 10, 2015): Nazareth Hospital has been recognized by The Hospital and Healthsystem Association of Pennsylvania (HAP) for exceptional readmission reduction results and participation as part of its Pennsylvania Hospital Engagement Network (PA-HEN). Nazareth Hospital was presented with the Readmission Immersion Project Award during the PA-HEN Capstone Celebration held in early March.
To qualify for the Readmission Immersion Project Award, Nazareth Hospital was required to demonstrate a continued reduction in hospital readmissions from the 2011 Immersion Project baseline established by HAP. Nazareth Hospital reduced readmissions by enacting a number of measures, including identifying patients potentially at risk for readmission and scheduling follow-up physician appointments for patients after discharge. For Nazareth Hospital, all-cause readmission rates decreased from 13.4 percent in 2012 to 10.4 percent in 2014.
In addition to the readmission reduction qualification, participating facilities must to take part in 75 percent of PA-HEN’s education, coaching and network events. Nazareth Hospital more than surpassed that figure by attending 95 percent of all events, as well as building its own improvement events.
