Physician-led Mercy Accountable Care works with participating physicians and payers to ensure that patients, especially the chronically ill, get the right care at the right time in the right setting, to improve quality, lower the cost of healthcare delivery and share in savings.

Mercy Accountable Care provides its physician partners access to the tools and support they need to help them maintain independence and succeed in achieving better outcomes, better care and lower costs. These tools include comprehensive data analysis; a robust ambulatory care coordination program that includes a variety of healthcare professionals, such as nurse care managers, social workers, community health workers, behavioral health specialists, and health coaches; and access to a diverse, integrated healthcare delivery system.

Mercy Accountable Care, LLC, participates in Trinity Health Integrated Care (THIC)—a Medicare Shared Savings Program (MSSP) Track 3 accountable care organization (ACO). THIC serves patients in Idaho, Indiana, New York, Oregon and Pennsylvania. THIC focuses on improving patient experience; improving the health of its patients; and lowering the overall cost of care.
