Our clinicians have an average of more than 15 years of experience. Licensed Registered Nurses (RNs) develop an individualized healthcare plan to promote maximum independence and health at home. Licensed Practical Nurses (LPNs) assist RNs in the provision of quality and compassionate care.

The home health care plan may include:

Health Management Programs

Mercy Home Health's comprehensive health management programs are designed to improve self-management of chronic health issues. Staff teaches patients to quickly recognize problems and when to contact their doctor to help prevent re-hospitalization due to:

  • Congestive Heart Failure (CHF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Diabetes self management/nutrition

Ostomy, Incontinence and Wound Care Nursing

Registered Nurses certified in Wound, Ostomy and Continence (WOCN) provide specialized care and patient instruction for those with urinary and bowel ostomies and urinary incontinence. We can also evaluate and recommend treatments for hard to heal wounds.
