The No. 1 goal of Mercy Home Health’s Telehealth monitoring program is to keep people with chronic illnesses out of the hospital. Equipment placed in the patient’s home electronically transmits data to skilled clinicians who constantly monitor patients’ vital signs 24 hours a day, seven days a week.
Those who benefit most are patients with ongoing health problems such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), or diabetes, explains George Peto, RN, Telehealth Supervisor. The program has proven to reinforce positive behavior and lifestyle changes as well as provide patients and family’s peace of mind knowing a medical professional is constantly monitoring their health and responding immediately if issues arise.
“Patients need constant monitoring of blood pressure, heart rate, and weight, especially if they are living with a chronic illness,” he says. “If our nurses see something outside the norm, they call the patient. If needed, we contact the doctor and send a nurse to the home.”
Telehealth equipment includes a scale, blood pressure cuff, and pulse oximeter, which checks blood oxygen levels. The wireless, easy-to-use equipment sends data via the patient’s landline phone or a cell phone.
“The goal is to keep patients safe at home and to decrease unnecessary hospital visits or stays,” Peto says.
