Telemonitoring manages chronic illness at home

Remote telemonitoring offers daily health check-in with experienced clinicians

This in-home electronic equipment wirelessly sends vital signs such as weight, blood pressure, heart rate and blood oxygen level to Mercy Home Health clinicians every day. With daily monitoring and one-on-one follow-up when needed, Mercy Home Health’s Telehealth program has a proven success rate at cutting down on repeat, unnecessary hospital visits, adds Ruth Martynowicz, MBA-HCM, BSN, vice president of operations at Mercy Home Health.

“Our average 30-day hospital readmission rate for Mercy Home Health patients receiving Telehealth monitoring is as low as 5.8 percent, as compared to the national average of about 25 percent,” she says.
