Congestive Heart Failure Program

Congestive Heart Failure is one of the top chronic illnesses in the United States and is a leading cause for hospitalization for people over 65. The team of cardiac specialists at Mercy Health System constantly is working to optimize care for heart failure patients.

Mercy participates in the American Heart Association’s Get With The Guidelines—Heart Failure Quality Improvement Program to enhance patient outcomes. Our heart failure care program includes educating patients and enrolling them in our in-home telemonitoring program to help manage symptoms of the disease.

In 2013, Nazareth Hospital earned The Joint Commission’s Gold Seal of Approval™ for its heart failure care program. Mercy Fitzgerald, Mercy Philadelphia and Nazareth Hospitals are each recipients of Get With The Guidelines Quality Awards for heart failure care.
